Required Registration Documents

To register your child, you will need to scan or take a picture of the required documents listed below and save each document individually to a file on your computer or phone. All required documents/forms must be uploaded into our online registration platform. No scanner? Click here for help.

PROOF OF RESIDENCY:

Agreement of Sale, Deed, or Tax Bill; OR
Current Lease/Lease Renewal; OR
Notarized Affidavit of Residency* (ENG / SPAN) - (Needed if living with a Clementon resident) AND Notarized Sworn Statement of Landlord* (ENG / SPAN) - (Needed if living with a Clementon resident who doesn't own the home)

AND

Two Parent/Guardian Bills - Must be a full copy of the page showing recent date, your name, and Clementon address (no envelopes or partial copies accepted); OR
30-Day Bill Extension Request* (ENG / SPAN) - (Needed if you are new to Clementon and haven't received two bills yet)

AND

Parent/Guardian Photo ID

AND

MEDICAL / OTHER:

Physical Examination Form* (ENG) – (or upcoming appointment confirmation from doctor's office)
Immunization Records
Child’s Birth Certificate
Transfer Card and Recent Grades (if applicable)
Individual Educational Program (IEP) or Early Intervention Service Plans (IFSP) – (if applicable)
Custody Papers – (if applicable)
Medication Dispensing Forms* – (if applicable):
- Prescription Medication Dispensing Form (ENG & SPAN)
- Asthma Treatment Plan (ENG)
Dental Exam Form (Recommended for Kindergarten) (ENG / SPAN)
